Supreme Court Forms
All 29 forms prescribed by the Supreme Court Rules 2012: the notice of appeal, application for leave, objection to competency and cross-appeal, the constitutional application and reference forms, the review forms including election petition reviews, the Listings Rules forms, appearance and address for service, and the two Second Schedule forms for prisoners appealing without a lawyer. Each is available in Microsoft Word and PDF.
Order 1 Rule 8 of the Supreme Court Rules 2012 provides that the forms in the First Schedule shall be used where applicable, and that a document is sufficient if it is substantially in accordance with the form or has only such variations as the nature of the case requires. The forms below reproduce the First and Second Schedules as consolidated in the pngInLaw edition of the Rules (November 2020 update, incorporating the 2013 amendments and the Listings Rules forms 10A to 10D), with dotted lines added where the original leaves a space. The wording is otherwise unchanged.
Download the Word version to type into it, or the PDF to print and complete by hand. Every document a party files must carry the address for service in Form 17, and a party served with an appeal or application who wants to be heard files an appearance in Form 16. The file number prefix tells the Registry what kind of proceeding it is: SCA for appeals and applications for leave, SCRA for criminal appeals, SCM for appeals by notice of motion under Order 10, SC Rev for reviews under section 155(2)(b), SC Ref for constitutional references, SCOS for originating summonses and SCAPP for bail, extension of time and section 57 applications. The Rules are amended from time to time and the Registry issues practice directions. Filing fees are set separately, and an election petition review carries a K750 filing fee and a K5,000 security deposit under Order 5.
