National Court Forms

Order 2 Rule 21 of the National Court Rules provides that where a form is prescribed for a document, the document shall be in that form or a form to the like effect, with such variations as the circumstances require. The forms below reproduce Schedule 1 as consolidated in the pngInLaw edition of the Rules (November 2020 update), including the human rights forms added by Order 23 in 2010, with dotted lines added where the original leaves a space. The wording is otherwise unchanged. Forms 1 to 3 are specimen headings using the Rules’ own illustrative party names; replace them with the parties’ names.
