Association Forms
The two paper forms prescribed by the Schedule to the Associations Incorporation Regulation 2026: Form 1, the notice of intention to apply for the incorporation of an association (section 6(1) of the Act), and Form 2, the consent of a committee member (section 23(1)). Each is available in Microsoft Word and PDF.
Regulation 4 of the Associations Incorporation Regulation 2026 prescribes only two paper forms. Every other form under the Associations Incorporation Act 2023, including the application for incorporation, the annual return, notices of change of committee members, public officer, rules, name and registered office, the re-registration application and the overseas association forms, “is comprised of the data fields contained in the online registry for the form in question” and is completed on the Registrar’s portal at the Investment Promotion Authority. The forms below reproduce the Schedule, with the fields set out as fill-in tables and the draft’s stray reference to an “Associations Act 2020” in the certification rendered as the Associations Incorporation Act 2023. The wording is otherwise unchanged.
The Regulation from which these forms are taken is a draft dated 8 May 2026. It had not been gazetted when this page was prepared, and it is not on PacLII. The final forms, the fee schedule and the online-only filing rules may differ. Confirm the current form and fee with the Registrar of Companies before lodging anything. Download the Word version to type into it, or the PDF to print and complete by hand. Form 1 asks for full legal names (initials are not enough), month and year of birth, nationality, residential addresses without PO Boxes, and postal addresses for the applicant, up to five committee members, any shadow committee member and one or two public officers, then the association’s own addresses, principal activity, objectives, category and the two statutory statements. It must be lodged with a copy of the proposed rules and any trust deed and the prescribed fee; the Registrar then approves a shorter public notice for the newspaper. Form 2 is not filed with the Registrar: each committee member signs one before appointment and the association keeps it, producing it on request. Forms completed on the online registry
The following filings have no paper form. Under regulation 4(1) they consist of the data fields on the online registry, which the Registrar may make the only permitted channel (regulation 2(2)). Where the Act sets no filing period, regulation 3 requires filing within one month of the event.
